If you wish donate the altar flowers for any date or occasion, please fill out the form to the left detailing the necessary information:  Your name, email, the date requested for altar flowers (or if you like any open date), and what specifically you'd like the bulletin to say regarding the dedication of the altar flowers. Please include your phone number.  You will be contacted either thru email or via phone to confirm your request.  The cost is $40.00, payable to the church, and should be dropped off at the church office once the date is confirmed.
